HOW WE CARE

Care that fits the person

We care for adults who need daily help but do not want to live in a large facility. Residents receive personal care, medication support and help with daily routines, along with the privacy, family visits and small comforts that make a place feel like home.

We welcome Medicaid (Oregon APD) and private pay residents.

What daily support includes

Care plans are tailored to the person and to what each home is licensed to provide, and they can change as needs change.

How admissions works

Every admission is a little different, depending on the person and on whether care is paid through Medicaid or privately. Most follow these steps.

1. Reach out

Call us or send a request. Tell us about the person and the support they need, and share their clinical information.

2. We review for fit

We review care needs to see whether we can safely meet them. If we are not the right fit, we will tell you honestly.

3. Tour and screening

Visit a home, meet the people who provide care and ask anything on your mind. We also complete the screening the State of Oregon requires.

4. Paperwork and move-in

We go over the residency agreement and house policies, set up medication and care orders, and arrange a move-in date.

Medicaid and private pay

We welcome both Medicaid (Oregon APD) and private pay residents. For Medicaid residents, a state or county case manager is part of the planning. For private pay, we will go over costs and the residency agreement with you, and you have the right to ask for a long-term care assessment.

Call us and we will talk through which path fits your situation.
