High-Acuity Care in Salem, Oregon
Attentive care for people with significant physical needs, in a small home with caregivers onsite around the clock.
What high-acuity care looks like in a small home
High-acuity care means a person needs a lot of hands-on help or close monitoring. That can include help with most daily tasks, transfers that need more than one caregiver, complex medications, or several health conditions at once.
What families should know
Assessment comes first
We review the clinicals and meet your loved one before deciding whether we can meet their needs, and we tell you honestly.
Care rates follow needs
Care rates use tiers from T1 (lowest need) to T5 (highest need). We explain what applies before admission.
Equipment and supplies
Medical equipment is usually arranged through the doctor, home health or a supplier, and we help coordinate.
Questions to ask any home about high-acuity care
- Can you care for someone who needs two caregivers or a lift for transfers?
- How are complex medications managed and checked?
- Who is the nurse, and how often do they visit?
- How quickly can you reach a doctor when something changes?
- What equipment can the home use?
- What would lead to a move, and how much notice would we get?
When a home may not be the right fit
Every home has limits. If a resident’s condition becomes complex, unstable or unpredictable beyond what the home can safely provide, Oregon rules allow a move with written notice. We review changes with you and the doctor early, and we tell you honestly if a nursing home would be a better fit.
Common questions
How do I know if my loved one is a fit?
Send us the clinicals. We review them, meet your loved one, and tell you honestly whether we can meet their needs. Our West Salem home specializes in higher-acuity care and dementia support.
Can residents with several health conditions live here?
Often yes, depending on the conditions and the care they need. We look at the whole person and review needs with you and the doctor.
How is this different from a nursing home?
A small home is more personal, with fewer residents and the same caregivers every day. A nursing home offers more medical staffing. The right choice depends on your loved one’s needs.
Want to talk about your loved one’s needs?
Tell us about them and we will tell you honestly whether we are a fit.
